1) Creating the login

   a) Please enter your basic details such as Name, Gender, Mobile, Email, Date of birth and Registration Type, Details of the registration type can be read from our Scheme Brochure

   b) Once this done the registration is complete and now you can login, using the registered mobile number and date of birth as password in ddmmyyyy format.

2) Login after the registration.

   a) Once this done the registration is complete and now you can login, using the registered mobile number and date of birth as password in ddmmyyyy format.

   b) Here you need to provide the full details of your profile including the photograph, Signature and certificates.

   c) You can SAVE the details and can log off in any stage. You need to fill at least the mandatory fields and payment details and then only able to CONFIRM the registration. Only CONFIRMED application will be considered for enrollment verification.

Things to be ready before submitting the online application

a) Doctor's Photo: Doctors need to upload passport size photo. Doctors will require a scanned/digital image of their photograph, dimensions 150 (Width) x 200 (Height) pixels (preferred) and file size should be between 10 kb-500 kb. (File formats: .jpg/.gif/.png)

b) Signature of Doctor: Doctors need to upload their signature. Doctor should also have a scanned image of their signature. The Signature should not be in capital letters. The doctor should sign on a white paper with black/blue ink pen. The preferred dimension for signature is 140 x 60 pixels and file size should be less than 200 kb. (File formats: .jpg/.gif/.png)

c) Certificates: The following certificate and documents to be uploaded in the website:

    i) Age proof: Need to upload a document having your age as proof; Aadhar. Pan card etc

    ii) IMA life member certificate

    iii) Eligibility Certificate

       Acceptable File formats: .jpg/.gif/.png/.pdf with a file size less than 1 MB

3) Payments

You can make the payment of registration through Online and offline modes

a) Online Mode: Just select the online and make the payment using the internet banking, Credit card, Debit card or UPI. Once Payment is done you will be redirected back to our websites generating the payment receipt.

b) You can also check the status of the each online transaction using the CHECK STATUS button in the TRANSACTION HISTORY section.

c) Offline Mode: You can make the payment with Cheque/DD/Bank transfer and each of this you select the mode of transaction done enter the DD/ Cheque /Transaction number with the date of payment in case of bank transfer or Cheque /DD date in the console.

d) In case of Offline transaction you need to send the Cheque/DD/Bank transfer receipt to our office mentioning your NFWS number, Name & Mobile number on the reverse side of DD/Cheque and online transaction receipt to our below office address by post.

e) Once we receive the Cheque/DD/Bank transfer to our office will submit to bank/Check the bank transfer details and subsequently if the amount is credited to our account then your account will be activated.

4) Confirmation of Registrant details

a) Once the mandatory fields and Payment details is entered then you need to confirm the registration.

b) Only confirmed registration will be taken for the enrollment verification.

c) On confirming if the payment is made by Cheque/DD/Bank transfer, then you have to send the payment details to our office.

d) Once the status is under verification you can see your profile details in the dashboard.
